My personal advice: If you don't want to always succeed in your actions, don't write about your actions in the perfect form. Write what you want to do or what your plan is. This is mostly relevant for combat scenes. Example: Instead of "I hit him in the face" you should write "I'm aiming in his face" or "I'm going to hit him in the face". That way the outcome of your actions will be more random.
